How to refer your patient
- Ongoing education and support for people with diabetes and/or partners and carers
- Telephone advice and support
- Monitor and manage blood glucose levels
- Education on medications including insulin therapy
- Pump therapy
- Initiating therapy or support ongoing
- Continuous Glucose Monitoring
The CDE clinic accepts referrals for patients 18+ years of age. Referrals, TCA (Team Care Arrangement), and EPC (Enhanced Primary Care plan) are accepted from GP’s and community-based health professionals, or patients can self-refer using self-referral form available from Medical Consulting Suites Specialist Clinics reception.
We do not currently offer Credentialled Diabetes Education support (CDE) and consultations for gestational diabetes, in-patients, home visits or to Aged Care facilities.
Please include the following information on the referral:
- Recent investigations including diabetes pathology – Urine ACR, HbA1c, Biochemistry / UEC, FBE, & Lipid profile
- Reason for referral
- Known complications or comorbidities (e.g. cardiovascular disease, kidney disease, retinopathy, cerebral vascular disease, nerve damage in the lower limbs, anxiety, depression, foot ulcers)
- Current and complete medication history (including non-prescription medicines, herbs and supplements)
- Functional impact of symptoms on daily activities including impact on work, study or carer role
- If the person identifies as an Aboriginal and Torres Strait Islander

Referrals which do not meet the Statewide Referral Criteria may be declined.
The demand on specialist clinics is high. Consequently, waiting times for some specialties can be quite lengthy. Should you have any queries regarding your patient’s referral, please contact Medical Consulting Suites Special.
This CDE specialist clinic is a Tier 2 non-admitted service. There is no out of pocket fee for this clinic.
