Referral Information
To improve access and appropriateness of referrals to public hospitals, the Department of Health and Human Services (DHHS) has developed statewide referral criteria for certain medical and surgical conditions.
Referring clinicians must comply with statewide referral criteria for all referrals – src.health.vic.gov.au
We accept GP and medical specialist referrals for all Specialist Clinics.
The standard information required on all referrals includes:
- Detailed reason for referral
- Working diagnosis
- Medical history—current and relevant past
- Medications
- Allergies
- Relevant investigations
- Relevant social history
- Current management of the condition and response to this
- Patient contact details, including need for interpreter

All referrals will be triaged according to clinical need by the specialist required for your patients condition.
The demand on specialist clinics is high, Consequently, waiting times for some specialties can be quite lengthy.
Referrals that do not meet the statewide referral criteria are likely to be delayed in processing, resulting in delays of the patient being seen. Patients will not receive an appointment or be placed on an appropriate waitlist until a com-plete specialist clinic referral has been received. If a referral is declined due to being incomplete, the referring clinician will receive correspondence explaining why the referral has not been accepted and the referring clinician must resubmit the referral again in full.
