The Geriatrician Clinic provides specialist assessment and management of cognitive disorders, including dementia and its behavioural and psychological symptoms. Services include comprehensive geriatric assessment, frailty management, falls prevention, and the promotion of preventative health strategies for older adults.
How to refer your patient
We accept referrals from GP’s, Nurse Practitioners and Medical Specialists.
Referrals must comply with Statewide Referral Criteria.
The standard information required on all referrals – src.health.vic.gov.au
Referrals are triaged by the specialist according to the patient’s clinical need. Providing comprehensive and relevant clinical information will assist the specialist to accurately assess the referral and determine the appropriate priority.
Please include the following information on the referral:
- Past medical history
- Active medications
- Any Allergies
- Full bloods (FBC, EUC, CMP, LFT, B12, folate, iron studies, TFT, vitamin D and Hba1c) to be done within last 6 months
- Brain imaging to be done within last 12 months (CT Brain or MRI Brain)

Referrals which do not meet the Statewide Referral Criteria may be declined.
Waiting times and referral enquiries:
The demand on the Geriatrician Clinic is high. Consequently, waiting times for this Clinic can be quite lengthy.

Fees
The Geriatrician Clinic is a public clinic and no out-of-pocket fees apply for eligible patients.
Patients who are not eligible for Medicare or DVA-funded services will incur fees.
