For clients at home inclusive of Residential aged care in Campaspe shire including Moama and Cohuna.

– For comprehensive symptom assessment and management

– Referral and coordination to other allied health such as physio/ Occupational therapy/ dietician/ social work

– Complex psychosocial and spiritual needs of patient and or family/carer

– End of Life Care for patients wishing to die at home

– Coordination of care between services

– Coordination with General Practitioners and Medical Specialists

– Supply of equipment

– Bereavement support.

– after hours advice through ERH after Hours Bed coordinator
