Services
Post Acute Care provides support and care for patients to recover at home after they leave a Victorian Public hospital. The service is available for up to 28 days after your hospital stay.
This service is for patients who have limited support at home. Patients on a home care package are not eligible.
Services include:
- Homecare
- Personal care
- In Home Respite
- Shopping assistance
- Community Nursing
Referrals
The service requires consent from you or your carer.

Fees
The service is free, however patients may be required to contribute to the cost of some items (such as wound dressings).
