Residential in Reach (RIR) is a service designed to improve the care of people living in Residential Aged Care Facilities (RACFS) in Echuca and Moama. The RIR nurse provides assessment and short term support to residents with acute medical conditions and may provide an alternative option for transfer to the Emergency Department where appropriate. A plan of care is developed with the resident, their GP and facility staff to provide early intervention and ensure that care is delivered in the right place at the right time.
- Comprehensive nursing assessment
- Urinary catheter care
- Intravenous antibiotic treatment
- Dehydration management
- Wound care
- Providing support and education following a residents ED or hospital discharge
Who is the program for?
The program is for any permanent resident of an Aged Care Facility in Echuca or Moama.
The service requires consent from you, your GP or a family member.
Referrals are received by phone, fax or email.
Your doctor, the Aged Care Facility staff or a family member can request the service.
The service is free.
The RiR nurse works with your doctor and the staff at your Aged Care Facility.
The Aged Care staff continues to care for you as usual.
