Services
Residential in Reach helps to improve the care of people living in Residential Aged Care Facilities in Echuca and Moama. The Residential in Reach nurse provides assessment and short term support to residents with acute medical conditions and may help a transfer to the Emergency Department. A plan is developed with the resident, their GP and facility staff to provide support by providing care in the right place at the right time.
Services include:
- Comprehensive nursing assessment
- Urinary catheter care
- Intravenous antibiotic treatment
- Dehydration management
- Wound care
- Providing support and education following a resident’s ED or hospital discharge
Who is the program for?
The program is for any permanent resident of an Aged Care Facility in Echuca or Moama.
Referrals
Referrals can be made by residents, family members, GPs or the Aged Care Facility staff.
