What we do
Credentialled Diabetes educators provide support and self-management education to people with diabetes 18 years and over.
- Ongoing education and support for people with diabetes and/or partners/carers
- Telephone advice and support
- Monitor and manage blood glucose levels
- Education on medications including insulin therapy
- Pump therapy
- Initiating therapy or support ongoing
- Continuous glucose monitoring
How to access this clinic
You will need a referral letter from your GP, medical practitioner or you can complete a self referral form to access this service – ERH Diabetes Self Referral Form (DOC).
All referral must include medications list and recent diabetes pathology – Urine ACR, HbA1c, Biochemistry / UEC, FBE, & Lipid profile.
Your doctor will need to fax or send it electronically your referral, or you can bring the referral into the clinic. When your referral is received it will be reviewed by the Credentialled Diabetes Educator and categorized based on clinical urgency.
You will then be notified of your clinical urgency category and expected wait time for your appointment. The demand on specialist clinics is high. Consequently, waiting times for some specialties can be quite lengthy.
What to bring
- Your Medicare care
- A list of medications you are currently taking including herbal supplements and vitamins
- Any relevant test results
- Your blood glucose diary/record book
- Your diabetes medicine
- Your favorite hypo medicine
- If you have an insulin pump download your pump before you come to your appointment.

Our Specialist Clinics is located in Echuca Regional Health, Building E, 25 Francis street Echuca.
There is 2-hour parking on-street parking in Francis St and Leichardt St. Clearly signed disabled parking is also available.
A text message reminder will be sent to your mobile the day prior to your appointment.
Please present to main reception desk on arrival to let staff know you have arrived.

Fees
