Services

The Transition Care Program (TCP) provides low level therapy and care for up to 12 weeks for people over the age of 65. People can be admitted to the program in either a residential bed in the Rehabilitation Ward or receive therapy in their own home. Admission to the program must follow a hospital stay. The program allows people to take more time in a non-hospital environment to complete their rehabilitation or to achieve the best possible recovery. TCP can also support families to finalise long term care arrangements, including returning home with supports and residential placement. The program provides therapy which may include physiotherapy, occupational therapy, speech pathology, nursing support, social work, podiatry , dietetic advice and Case Management.  In addition, services provided to community-based clients may include personal care, home care and in-house respite. The ERH program is part of a formal partnership with Bendigo Health Care Group. ERH currently has six residential places and six community based places.
