HITH provides nursing care for people at home who would otherwise be hospitalized with acute but stable medical conditions.

Each patient is assessed to determine his or her needs, and a plan for the care is agreed. This sometimes involves several health professionals.

While on this program, the client is considered an inpatient of Echuca Regional Health, and program guidelines apply to ensure patient safety. There is no cost to the client for this treatment.

HITH aims to:

• Allow patients to receive treatment in the familiar surroundings of their own home
• Minimise the disruption to the patient’s regular routine during their treatment and empower the patient and their family
• Minimise family separation associated with hospital admission
• Co-ordinate care from hospital to home and monitor progress
